Documents to be submitted with the membership application:

  1. A copy of the academic certificate recently certified by the legally authorized authorities, or a recent original graduation certificate.
  2. A statement of completing the legal period with training under supervision in the teaching hospital (completion of the internship period)
  3. Four recent personal photos.
  4. A copy of the ID card.
  5. Certificate of clean criminal record.
  6. A copy of the receipt of payment of the registration and the prescribed subscription.
  7. A copy of the family booklet.
  8. A letter from the employer regarding good conduct and behavior upon renewing membership.
  1. Copies of the employment contract.
  2. A residency from the employer.
  3. A message of good conduct and behavior from the previous employer and the medical association in his country.
